Please fax a consult letter to our clinic:
For patients who were given an appointment at the time of discussion with the on-call surgeon or resident, consults should be faxed to (204) 787-8765.
For patients to be reviewed by the staff surgeon, fax to (204) 940-2670. If these patients have wounds (e.g crush injuries, nailbed injuries, amputations, burns, frostbite, etc), please send photos to both:

As well, please print the attached instruction sheet for the patient. It goes over how to find our clinic, what to expect, our contact information, and how to change their appointment if required.
